The inquiry has now resumed after breaking for lunch.
During this morning's session, inquiry counsel Jason Beer questioned Vennells on how many cases Second Sight were supposed to review as part of their investigations into possible "systemic defects". Stay with us as we'll bring you updates, analysis and reporting from inside the inquiry room.
Several documents showed that Vennells wanted the review to look at two or three cases. And remember you can press "play" at the top of the page to follow the inquiry live.
When Beer questioned how a systemic failure could be demonstrated from only a few examples, and Vennells went on to say: "I was not trying to close anything down."
