Four lorries, a car and a van were involved in the crash on the southbound carriageway between junction 6 and 7 at Spaghetti Junction and Great Barr. Two lorries crashed on the northbound carriageway before one crossed over the central reservation and overturned. It was then hit by several vehicles.
The M6 is shut northbound between junctions 6 and 7 and southbound from 8 to 6. Long delays were expected. The M6 is shut between junctions 6 and 7 northbound and from 8 to 6 soutbound.
West Midlands Fire Service said lanes should re-open soon. The southbound carriageway will be shut until 1900 GMT and the northbound carriageway until mid-afternoon, the Highways Agency said.
The fire service freed three people from their vehicles and six crews remained at the scene cutting free another motorist, a spokesman said. Outside lanes will remain closed for longer and long delays are expected.
One person was airlifted to Selly Oak Hospital by police helicopter.
