Health and Social Services Minister Mark Drakeford said the project shows the Welsh government's "commitment to putting people first".
"The new system will make it easier for these services to work together to deliver improved services and support for individuals, families and communities across Wales," he said.
Councillor Mel Nott, leader of Bridgend council, said he was proud the authority had been chosen to lead the project.
"It will lead to seamless information sharing between organisations and an improved health and social care service for the community," he added. |
