Avon and Wiltshire Mental Health Partnership said it would "work with colleagues in health and other partner agencies to give our full support in determining the series of event which led to this tragic death".

The hospital spokesman added access to the maternity unit is controlled by a "door release" and CCTV. It is not clear how Ms Bevan was able to leave.
