Police officers were reported to be looking for a 14-year-old boy said to have been unhappy at a suspension from the SuccessTech Academy in Cleveland.
Cleveland Mayor Frank Jackson told reporters that two teenage boys and two adults had been hurt in the shooting and another girl had injured her knee.
It remains unclear what has happened to the gunman. The school was evacuated.
He said police were holding the shooter and securing the school site.
Ronnell Jackson, 15, said he saw the shooter running down a hallway. "He was aiming at me, I got out just in time," AP news agency quotes him as saying.

Mayor Jackson told reporters that police were checking the school building floor-by-floor.
"We have no reason to believe there's more than one suspect," he said.
The incident comes six months after the US suffered its worst ever campus shooting, when a gunman killed more than 30 students and staff at Virginia Tech.
