The woman, who is believed to be called Sophie, is being treated for head and face injuries at the Royal Gwent Hospital in Newport. She is said to be in a stable condition.
It is thought her car left the road between Newport and Usk and landed in farmland which meant the car was not visible from the dual carriageway.
An elderly resident who lives close to the scene of the crash, said she heard a "large metallic sort of bang" just before 0200 BST on Tuesday morning.

The woman, who did not want to be named, said she believed something had gone into a nearby farm barn and drove to have a look but could not find or see anything so went back to bed.
"I came back feeling very uneasy so I feel very badly about it that I hadn't seen anything more than I did," she said.
"But I am very very pleased to hear the young lass Sophie is recovering because I had a very upsetting day feeling I should've done more."
The woman said a man from Newport, who shoots on nearby farmland, discovered the driver.
He told her he had not seen the injured woman or the wreckage initially and only saw the vehicle when opening a gate some distance away and looking back.
"He climbed up and found Sophie who had managed to crawl out of the car," she said.
"Apparently she grabbed his hand and was very reluctant to let him go.
"He comforted her and at the same time made a telephone call to the police and ambulance and so on."
The injured woman had gashes to her head and injuries around the eye, the man told the resident.
