Dr Kevin Cleary, Medical Director of the National Patient Safety Agency said: "Since 2003 the NPSA has been encouraging all NHS healthcare staff to report patient safety incidents to us so that we can identify common patterns and produce guidance accordingly. As a result, reporting has improved across all regions - this is shown in the 50% rise in staff reporting incidents in Wales from 2005 to 2008.
"An improving reporting culture is proof that healthcare staff are taking patient safety very seriously in Wales."
