A male passenger, from the Mold area, who was in a Vauxhall Corsa was taken to the Wrexham Maelor Hospital with life threatening injuries.
The driver of the Corsa, who also lives locally, suffered a serious leg injury and was taken to the Countess of Chester hospital.
A couple in the third vehicle involved in the crash, a Daihatsu, suffered minor injuries and North Wales Police said they are not believed to have needed hospital treatment.
Police said the teenager under arrest is also from the Mold area and he was driving a Peugeot car.
